The Exceptional Family Member Program is a mandatory enrollment program (MCO P1754.4A) for all active duty personnel. The EFM program is designed to provide assistance to service personnel with a special needs family member, before, during and after relocation due to Permanent Change of Station (PCS) orders. The program ensures that a sponsor will be assigned to a location where appropriate services exist to support the special needs of the family member. The family member can be a spouse, child, stepchild, adopted child, foster child or dependent parent

 
WHO QUALIFIES FOR EFMP

 

If you can answer “yes” to any of the questions below, contact your EFMP Coordinator for more information about enrollment.

 

  • Do you have a family member with diagnosed asthma, attention deficit disorder, cancer, lupus, heart disease, leukemia, diabetes, any other long term illness, or physical and/or developmental disability? 
  • Do you have a family member with any emotional disability such as mental illness, or clinically diagnosed depression?
  • Do you have a child in special education more than 20 percent of school time?
  • Do you have a family member in a residential treatment facility, receiving medical care through a state medical program, or in the incapacitated dependent program?
  • Do you have a family member receiving Supplemental Security Income (SSI) benefits?
 
HOW DO I ENROLL

 

  • Ensure your family member is enrolled in DEERS
  • Contact your local EFMP Coordinator for assistance and required forms
  • Return the completed forms to the EFMP Coordinator for processing

 

OPEN AND PRINT THE REQUIRED FORMS

 

  • DD Form 2792This form must be completed by ALL enrolees. No exceptions.
  • DD Form 2792-1 This form must be completed for ALL children in school with an IEP or IFSP attached if applicable.
